How did it end?

Longest time on hold, in minutes (optional)

How many different people or departments did you have to explain it to? (optional)

Enter 1 if the first person you spoke to dealt with it.

Your email

To confirm your wait and send you your record. If you're still waiting, we also email about once a month to ask. Deleted once your wait is finished. Never published or passed on.

I agree to this wait being stored and used in combined figures for this surgery, as set out in the privacy notice. It says something about my use of health services, and I can ask for it to be deleted at any time.

This is a true account of my own experience, or of someone I was with or caring for, to the best of my memory.

Dates and numbers only. We never ask what was wrong with you or who you spoke to.
