Log a wait

Rheumatology at York and Scarborough Teaching Hospitals NHS Foundation Trust. Somewhere else?

Good or bad, every wait counts. A quick wait tells us as much as a slow one.

What were you waiting for?

Date the wait started

The day you were referred, or told you needed this. Your best estimate is fine.

How did it end?

Date it happened, or you gave up

I gave up, or went elsewhere, without getting it.

I'm still waiting. Start my clock. We'll email about once a month to ask if you're still waiting.

Times the hospital cancelled or moved it (optional)

Enter 0 if it never did.

Times you turned up and were sent home without it going ahead (optional)

For example no staff, no bed or no equipment on the day. Enter 0 if it never happened.

How many different people or departments did you have to explain it to? (optional)

Enter 1 if the first person you spoke to dealt with it.

Your email

To confirm your wait and send you your record. If you're still waiting, we also email about once a month to ask. Deleted once your wait is finished. Never published or passed on.

I agree to this wait being stored and used in combined figures for this hospital specialty, as set out in the privacy notice. It says something about my use of health services, and I can ask for it to be deleted at any time.

This is a true account of my own experience, or of someone I was with or caring for, to the best of my memory.

Dates and numbers only. We never ask what was wrong with you or who you saw.
